required to call 2886 4234 on or before 31 Dec 2009 for enrolment.
2. After successful enrolment, Standard Chartered Bank (Hong
Kong) Limited (the "Bank") will debit the deposit
of the Health Check Up Promotion Price from the Cardholder's
designated Standard Chartered Credit Card or Standard Chartered
Co-branded Card Account (exclude Standard Chartered American
Express® Card) and such amount is not refundable (Basic
Check Up Plan: HK$228 / Female Check Up Plan: HK$728 / Comprehensive
Check Up Plan: HK$838 / Premier Check Up Plan: HK$1,223 / Female
Executive Check Up Plan: HK$1,388 / Male Executive Check Up Plan HK$1,988). Cardholder's
name and correspondence address and Participant's name (if applicable)

amend the terms and conditions at any time. All matters or disputes
will be subject to the final decision of the Bank and the participating
merchants. 12. If there is any inconsistency or conflict between
the English and Chinese versions of these terms and conditions,
the English version shall prevail. |
